W&J Sports Medicine Introduction Letter -- Please read before proceeding
The Washington & Jefferson College Sports Medicine Department strives to provide professional care and wellness to the health of all Washington & Jefferson College student-athletes. Our goal is to return our student-athletes to full participation as quickly and safely as possible.
All athletics paperwork must be completed/uploaded to ATS. Below are the directions on how to create an ATS profile as well as upload all required documentation. All forms must be completed by August 1st.Â
Please keep in mind that these forms below are not to be confused with the documentation required to also be completed by all incoming students for W&J Student Health Services. That form can be found HERE.
If you have ever been diagnosed or are currently under treatment for any of the following conditions, you MUST provide documentation (including a list of medications) from your physician that you are fully cleared to participate (no restrictions) in athletic activities:
- Heart murmur
- Any cardiac conditions
- Diabetes
Freshmen/Transfers/First-Year ParticipantsÂ
